All Getting Started Patients Assessments Photos & AI Staff & Admin Data & Reports
Reviva is a wound care platform that helps nurses and doctors track, measure, and manage wounds from a phone or tablet. It replaces paper notes and clipboards with digital records, smart photo analysis, and easy reports so your facility can provide better wound care.
Open the Reviva app on your phone or the website. You will see a sign-in screen. Enter the email address and password your facility gave you, then tap the green "Sign In" button. That's it — you will see your Dashboard right away.
On the sign-in screen, click the "Forgot Password?" link below the Sign In button. Enter your email address, and a password reset link will be sent to you. Click the link in the email to create a new password. If you still can't get in, contact your facility admin to set up a new password for you.
Go to the Patients page by tapping the "Patients" button at the top of the screen. Tap the green "Add" button with a plus icon. Fill in the patient's name, date of birth, and medical record number, then tap "Save." The new patient will appear in your patient list right away.
Tap a patient's name to open their record, then tap the "Wounds" tab. Find the wound you want to assess and tap the green "Assess" button on its card. Fill in the measurements, tissue types, and other observations, then tap "Save Assessment." Each assessment captures the wound's condition on that date.
A wound is the permanent record you create once — it stores the wound type, location, and onset date. An assessment is a snapshot at a specific point in time that records measurements like length and width, tissue types, pain level, and photos. One wound can have many assessments over days or weeks, so you can see how it changes.
While recording an assessment, tap the gray camera box labeled "Tap to take a photo." Your phone's camera will open — take a clear photo of the wound with good lighting. The photo will appear on the assessment. You can remove it and retake it if needed before saving.
Yes! Open a wound's gallery to see all photos arranged by date. You can use the opacity blend slider to fade between an old photo and a new one, which makes changes easier to spot. You can also play a slideshow that cycles through all photos at your chosen speed.
When you take a wound photo, the AI automatically analyzes it to identify tissue types and estimate the wound area. It highlights granulation (healthy red tissue), slough (yellowish tissue), and other areas. This helps you track healing trends without guessing by eye. The AI results appear right below your photo.
Only facility admins can add staff. Tap the green "Admin" button at the top of the screen (you will only see this if you are an admin). Under the Admin panel, go to the Users tab and tap "Add User". Enter their name, email (which will be their login), display name, and role — then send them their login details.
In the Admin panel, find the staff member you want to assign. Tap the "Assign Facility" option next to their name, then select the facility from the list. You can assign one person to multiple facilities if they work at more than one location.
The Dashboard is the first screen you see after logging in. It shows four big numbers: total patients, total wounds, total assessments, and active medications. Below that is a list of the most recent assessments and a risk overview that flags patients who may need extra attention.
The Analytics page shows charts and graphs that help you spot trends across your whole facility. You can see how wound sizes change over time, which types of wounds are most common, and how your healing rates compare to previous months. Use the date filters at the top to focus on a specific time period.
The Compliance section shows whether your facility is meeting documentation requirements. It checks that assessments are done on schedule, photos are taken regularly, and records are complete. A green checkmark means you are on track; a yellow or red warning means something needs attention.
On any patient's record or on the Analytics page, look for the "Export" button (it has a download icon). Tap it and choose your format — PDF for a printable report, Excel for spreadsheets, or Word for editable documents. The file will download to your device automatically.
Yes. After exporting a report in PDF, Excel, or Word format, you can share it directly from the app. Tap the "Share" or "Email" option that appears after export. Enter the recipient's email address, add a note if you like, and send. It works just like sharing a photo from your phone.
EMR integration means Reviva can talk to your facility's existing electronic medical records system. It uses standard formats like FHIR, HL7, and CCD to safely share patient data. Your IT team sets this up once, and then wound assessments and photos flow automatically between Reviva and your main system.
Admins can deactivate staff from the Admin panel. Find the person's name and tap the "Deactivate" or red button next to their profile. They will lose access to Reviva immediately, but their past patient records and assessments will stay in the system. You can reactivate them later if needed.
Only staff members at your facility who have been given access can see patient data. Each person's role determines what they can view or edit — for example, nurses can record assessments, while admins can manage staff. Reviva is HIPAA-compliant, meaning your data is encrypted and protected at all times.
Look for the small icon with an arrow pointing out of a door at the top right corner of the screen. Tap it, and you will be signed out and returned to the login screen. Always log out when you finish using a shared device to keep patient information safe.
On the Patients page, tap the search bar at the top. Type part of the patient's name or their medical record number (MRN). Results update as you type, so you will see matching patients instantly. You can also filter by facility using the dropdown next to the search bar.
Yes. Open a patient's record, select the wound, and scroll down to see a timeline of all past assessments. Each entry shows the date, key measurements, and any notes from that visit. You can tap any past assessment to view its full details and photo.
The ghost overlay lets you place a faint, semi-transparent version of a previous wound photo over your current view. This helps you line up the photo at the same angle and distance as the old one, making before-and-after comparisons more accurate. Use the slider to adjust how much of the old photo shows through.
If you take a photo on an iPhone, Reviva reads the camera information stored in the photo to calculate the wound's size automatically — no ruler needed. For other devices or if you prefer, you can also enter measurements manually using the length, width, and depth fields in the assessment form.
The Body Map shows a picture of a human body where you can place pins to mark where each wound is located. You select the wound and tap the body part where it is — like the left heel or sacrum. This gives you a quick visual overview of all a patient's wounds at a glance.
The Trajectory chart shows how a specific wound has changed over time. It plots measurements like area, depth, and tissue percentages on a graph so you can see healing progress at a glance. If the line is trending down, the wound is getting smaller — that is good news!
Yes! Reviva works on both phones and computers. On a phone, you can take wound photos right at the bedside. On a computer, you can view charts, export reports, and manage patients on a larger screen. Your data syncs instantly, so you can start an assessment on your phone and finish it on your computer.
Absolutely. Open a patient's record and you will see tabs for Comorbidities, Medications, and Medical History. Tap the green "Add" button on each tab to enter information. This helps you keep a complete picture of the patient's health, which can affect how their wounds heal.
The Payer Info tab stores insurance and billing details for a patient, such as their insurance provider and policy number. This information is helpful for billing departments and can be included in exported reports. You can add or update payer info from the patient's detail page.
There are three user levels: Super Admin (top-level, can manage everything and all users), Admin (can add staff, manage facilities, and see analytics), and Staff (clinical users who record assessments, take photos, and manage patients). Super Admins can create Admins and Staff. Admins can only create Staff. Your role determines which parts of the app you can access.
Your login email is what you type in to sign in — it's your username. Your display name is what other people see when they look at patient records (it shows in the "Created by" or "Updated by" field). You can change your display name without affecting how you log in.
The Admin panel (if you have access) has several tabs: Overview shows facility stats, Facilities lets you manage locations, Analytics shows charts and healing trends, and Users lets you add, edit, pause, or remove staff. Only Super Admins can see the Enterprise tab for cross-tenant management.
